Health History Form

Please fill out and return your Massage Intake Form at least 24 hours before your scheduled appointment. This will give me time to review it and prepare for your session in full. A health history form can be found in your confirmation email.

Arrival Time

Please arrive 10 minutes before your appointment time. This will give you time to transition from the business of life and prepare physically and mentally to relax and gain the full benefits of your massage. It will also give us time to discuss your health history form and any concerns you may have before your session begins. Arriving late will only take valuable time away from your massage as your session will still end on time. Unfortunately I am unable to extend scheduled sessions and you will be responsible for full payment regardless of arrival time.

Rescheduling Policy

I require a 24 hour notice if you would like to reschedule or cancel your appointment. Missed or cancelled appointments, with less than 24 hours notification, will incur full charges.
